1. Purpose of the Agreement

2. Entire Agreement

3. Services

3.1 The Contractor hereby agrees to provide massage therapy services under their license, certification, and professional qualifications.

3.2 The specific scope of services may vary depending on the Client's needs and will be discussed before the commencement of services.

4. Compensation

4.1 The Client shall pay the Contractor a fee of (state the amount) per (state the basis) for services rendered.

4.3 No other form of payment shall be accepted.

5. Hours and Availability

5.1 The Client shall make appointments with the Contractor that are mutually agreed upon by both Parties.

6. Cancellation

6.3 If the Client cancels an appointment, a new appointment shall be scheduled depending on the Contractor's schedule availability.

6.4 If the Contractor cancels an appointment, the Contractor shall notify the Client and either reschedule or provide a refund.

7. Miscellaneous

7.1 Representations & Warranties

7.1.1 The Contractor is the sole business owner that provides massage therapy services under this Agreement.

7.1.2 The Contractor has the necessary licenses, certifications and professional qualifications to provide massage therapy services under the laws of (state the jurisdiction).

7.1.3 The Contractor shall at all times comply with applicable laws and regulations.

7.2 Liability

7.2.1 The Contractor shall not be liable for claims, damages, or losses caused by the Client's negligence or willful misconduct.

7.2.2 The Contractor is responsible for any claims, damages, or losses caused by their failure to comply with applicable laws and regulations.

7.3 Dispute Resolution

7.3.1 To cultivate peace, when encountering any contention, it is crucial that both sides strive for shared comprehension and actively pursue peaceful solutions.

7.3.2 If the dispute escalates, it shall be under (state what the Agreement shall be construed and governed by).

7.4 Severability

7.4.1 If any provision of this Agreement is held by a court of competent jurisdiction to be invalid or unenforceable, then such provision shall be rephrased to match the interests of the Parties.

7.5 Confidentiality

7.5.1 The Contractor agrees to maintain the confidentiality of all records, services and other materials about the Client's personal or professional business (the "Confidential Information").

7.5.2 The Contractor further agrees not to disclose or use any Confidential Information for any purpose other than providing Services under this Agreement.

7.6 Amendment

7.6.1 This Agreement can only be amended or modified if both parties agree to and sign off on the changes in writing.

7.6.2 No spoken changes shall be accepted as valid.

7.7 Force Majeure

7.7.1 Any event of force majeure that prevents either Party from fulfilling their contractual duties, shall not be regarded as a breach of contract.

7.7.2 Force majeure is a concept that includes extraordinary events such as divine interventions, wars and armed conflicts, pandemics, civil unrest and other natural disasters.

7.7.3 In any event of force majeure, either Party shall be excused from their obligation for as long as said conditions last.

7.7.4 After the consequences of force majeure have been addressed, all involved Parties must return to their designated roles.

7.8 Jurisdiction

7.8.1 This Agreement shall be construed and governed under (state what the Agreement shall be construed and governed by).
